*Immunization Requirements For 7th Graders*

ALL 7th graders ( including Virtual students) MUST HAVE the following immunizations prior to the start their 7th grade year. (This is a state law requirement .)

1. Tdap-1 dose

2. Meningitis (MCV4)- 1 dose

*You may send in a copy of the shot record at any time after they have received these. You do not have to wait until the first day of school.

Mobile Dental Clinic will be at JMS on

Feb 11th and 12th of 2025

  • The Mid-Ohio Valley Health Department Mobile Dental Clinic will be at Jackson Middle School providing the following services: cleanings, fluoride, sealants, x-rays, and silver diamine fluoride treatments.

  • A consent is required in order to receive services.

  • Students who DO NOT HAVE A DENTIST OR WHO HAVE NOT BEEN TO THE DENTIST IN A YEAR OR MORE may participate. Please contact the school nurse to request a consent form to be sent home with your child or print a dental clinic consent from the link above, complete the consent, and return to the school nurse by dropping it off at school, sending in with your student, or email to candrews@k12.wv.us. They will accept Medicaid, private insurance, and have a sliding scale for children without insurance. NO CHILD WILL BE TURNED AWAY!
